Please read these Terms of Service carefully before using our website or services. By booking an appointment, purchasing a package, or using this website, you agree to be bound by these Terms. If you do not agree, please do not use our services.
Perle Physical Therapy & Pilates, LLC ("Perle," "we," "us," "our") is a licensed physical therapy and wellness practice located at 355 Porter Ave., Ocean Springs, MS 39564. We provide outpatient physical therapy, restorative Pilates instruction, dry needling, group classes, and related wellness services. These Terms apply to all services offered in person and through this website.
You must be at least 18 years of age to book services or purchase packages through this website. Clients under 18 may participate in services only with the written consent of a parent or legal guardian, who assumes full responsibility for the minor's participation and agrees to these Terms on the minor's behalf.
Perle is a cash-pay practice and does not bill insurance directly. All fees are due at the time of service or at the time of purchase for packages and memberships. Prices are subject to change with reasonable notice. Class packages and memberships are non-refundable once purchased, except where required by applicable law. Monthly memberships are billed on a recurring basis and require a minimum commitment period as disclosed at the time of purchase; cancellation requests must be submitted in writing before the next billing cycle to avoid additional charges.
We require at least 24 hours' notice to cancel or reschedule any individual appointment or private session. Cancellations made with less than 24 hours' notice, or missed appointments (no-shows), may be charged the full session rate or result in forfeiture of a session credit. Group class cancellations must be made at least 2 hours before the scheduled class time; late cancellations or no-shows will result in forfeiture of the class credit. We understand that emergencies happen — please contact us directly and we will do our best to accommodate you.
Physical therapy, Pilates, and exercise carry inherent physical risks, including the possibility of muscle soreness, strain, or injury. By participating in our services, you voluntarily assume all such risks. You represent that you have disclosed all relevant medical conditions, medications, recent surgeries, and physical limitations to your therapist or instructor before beginning services. You agree to consult with a physician before beginning any new exercise or treatment program if you have any doubt about your readiness to participate. Perle is not responsible for adverse outcomes resulting from your failure to disclose relevant health information or from participation against the advice of your therapist or physician. Nothing on this website constitutes a diagnosis, treatment plan, or medical advice.
To the fullest extent permitted by applicable law, Perle Physical Therapy & Pilates, LLC, its owners, employees, and contractors shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of our services or this website. Our total liability for any claim arising from a service you purchased shall not exceed the amount you paid for that specific service. Nothing in these Terms limits our liability for gross negligence, willful misconduct, or any liability that cannot be limited by law.
Perle does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Our services are not a substitute for emergency care, urgent care, or evaluation by a physician for acute conditions.
Perle does not participate in any insurance networks and does not submit claims on your behalf. Upon request, we will provide a superbill (an itemized receipt with appropriate billing codes) that you may submit to your insurance carrier for potential out-of-network reimbursement. We make no representation or guarantee regarding reimbursement amounts, eligibility, or your insurer's acceptance of any claim. Responsibility for all fees remains with you regardless of insurance outcome.
Access to on-demand video content is granted for personal, non-commercial use only. Your access is non-transferable and may not be shared with or resold to others. Exercise content carries the same physical risks described above; you assume all risk when performing exercises from our videos without in-person supervision. We reserve the right to update, modify, or discontinue on-demand content at any time without refund for unused access.
Perle reserves the right to decline or discontinue services to any individual at our discretion, including where continuing services would pose a safety risk or where a client's behavior is disruptive or abusive to staff or other clients. In such cases, any unused prepaid session credits will be refunded.
All content on this website — including text, images, videos, program materials, logos, and design — is the property of Perle Physical Therapy & Pilates, LLC and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, or create derivative works from our content without prior written permission.
We may update these Terms from time to time. When we do, we will update the date at the top of this page. Continued use of our services after changes are posted constitutes your acceptance of the revised Terms. We encourage you to review this page periodically.
These Terms are governed by and construed in accordance with the laws of the State of Mississippi, without regard to conflict of law principles. Any dispute arising under or related to these Terms shall be resolved exclusively in the state or federal courts located in Jackson County, Mississippi, and you consent to personal jurisdiction in those courts.
